It is intended to emphasize that informed consent is an educational process and a conversation between a lay person and a health-care professional—not merely the signing of a form that may not be understood or even read. The roles of the physician directing the phlebotomy (an invasive procedure) in the blood center, the transfusion service medical director in the hospital, and the treating physician are reviewed. Options for a separate consent document vs one that is part of an overall facility document are discussed.
