The 1st edition of Standards for Emergency Prehospital and Scheduled Out-of-Hospital Transfusions (EPSOHT Standards) incorporates new prehospital concepts while updating the standards that previously appeared as the 1st edition of Standards for Out-of-Hospital Transfusion Administration Services, released by AABB in 2018.

EPSOHT Standards will maintain and enhance the quality and safety of services provided by prehospital transfusion services (EMS services) and out-of-hospital transfusion services (infusion centers, nursing homes, long-term care facilities, hospice or home care settings).

This 1st edition becomes effective July 1, 2025 and will have a 2-year effective period.

Significant changes in this edition include:

Chapter 5, Process Control, has been split into two sections, one for prehospital activities, and one for out-of-hospital activities. All standards in Chapters 1-4 and 6-10 and 5.0 – 5.1.9.3 5 apply to both settings.
New Standard 5.4.4 ensures that out-of-hospital programs have two determinations of the recipient’s ABO group.
New Standard 5.8.2.1 requires that programs performing out-of-hospital activities ensure that patients maintain their patient identification attached through the transfusion.
New Standard 5.12 (et seq) focuses on the administration of blood in the prehospital setting.
New Standard 5.13 (et seq) focuses on requirements for the use of uncrossmatched blood in the prehospital setting.

Guidance for Standards for Emergency Prehospital and Scheduled Out-of-Hospital Transfusions includes verbatim requirements from the 1st edition of EPSOHT Standards interspersed with guidance. Language from the EPSOHT Standards that will maintain and enhance the quality and safety of services provided by prehospital and out-of-hospital transfusion services is presented; when there is appropriate guidance for that standard, the guidance language follows the standard with the heading “Guidance.” This format allows the reader to see both the requirement and the associated guidance side by side.

Please note that there is no guidance for some standards. The Prehospital and Out-of-Hospital Standards Committee has chosen to focus on those issues that may require additional context and/or examples to assist in attaining compliance with the EPSOHT Standards.
